Concussion Information

The New Baltimore Parks & Recreation Department is committed to the safety of all the athletes and participants in our classes and programs. Effective immediately, we will be following the state mandated concussion laws that prohibit recreation participants from returning to the activity until they receive written clearance from a doctor after being cleared of medical symptoms.

Below, you will find some helpful links that will get you and your family more familiar with the concussion law, give you more information about concussions and their symptoms, and also the Parent/Athlete Acknowledgement Form which MUST be signed and returned to the New Baltimore Parks & Recreation Department in order to participate in many of our classes/programs.

At this current time, we require this form to be filled out by parents/participants of the following activities:

~Heads Up Concussion Training is required for all coaches of our youth sports. Please complete the 25 minute online training video and submit your certificate of completion to the parks & recreation department.
